This form is for your convenience. All adults who plan to occupy the home must be listed on the application. We do require a non-refundable $45 application fee that can be paid securely on-line. After you submit your on-line application you will be directed to a secure processing page and prompted to enter your credit/debit card payment information. If you would also like to hold a home while we process your application please call the office at 614-433-9100. Applications will not be processed until the application fee and Authorization Form are received.

If you are approved for a home you will be required to provide additional documentation to verify the information you state on your application. If you are not comfortable completing the application online you may click here for a PDF version of the application that you can complete and email/fax/mail to us with your non-refundable $45 application fee. We accept and you will be prompted to enter your card information on a secured page after you submit the below application.
